LCSL Community Team Commitment Form
Each ommunity mustPlease complete this form and email it to the League Scheduler (lcslscheduler@yahoo.com) prior to July meeting (Fall) and February meeting (Spring).
LaGrange Registration Form
This is the form for LaGrange Players to register within the LYSA program ONLY.
Oberlin Registration Form
Please complete this form and mail to MPO Box 424, Oberlin, 44074.
New and returning players, U-9 and older, must include a photo for player passes.
New players for U-9 and up must submit two copies of the player’s birth certificate.
